<p>Pleural mesothelioma is the most common form of mesothelioma by far and accounts for 75% of all mesothelioma cases.  Pleural mesothelioma affects the respiratory areas of the body such as the lungs.  More specifically, the cancer attacks the lining of the lungs and ribs called the pleura hence the name pleural mesothelioma.</p>
<p>The main cause of pleural mesothelioma is exposure to asbestos.  Only a couple of months of being exposed to the deadly dust and fibres of asbestos without protection can lead to pleural mesothelioma thirty to fifty years later.  Elderly men of ages sixty to seventy are those most at risk to asbestos because this was the generation which worked with asbestos with little protection from the dust and fibres.  Due to the long latency period of pleural mesothelioma (30-50 years), these men are only just discovering they have the deadly cancer.  Currently, many of these men are lodging million dollar lawsuits against the companies who exposed them to asbestos.  Asbestos fibres get into the lining of the lungs by inhalation and become lodged inside the lungs.</p>
<p>The symptoms that are associated with pleural mesothelioma include persistent coughing, difficulty swallowing, facial swelling, weight loss, fever, rasping and coughing up blood.  Patients may additionally experience shortness of breath because as the tumour on the lining of the lungs expands, this leaves less room for the lungs to function properly.  Some patients also begin to feel severe pains in their chest and this is due to the spreading of cancerous cells.  A lot of these symptoms are similar to diseases and conditions which are far more common than mesothelioma which means that pleural mesothelioma is often very difficult to diagnose until it is too late.</p>

Recognition of pleural mesothelioma as a separate disease entity started as recently as the 1960s.</font></div>
<p>Pleural mesothelioma patients treated with both cisplatin and raltitrexed may survive longer than patients who receive cisplatin alone, according to a recent study conducted in Europe and Canada (J Clin Oncol. 2005 Oct 1; 23(28): 6881-9). Raltitrexed (Tomudex) works by interfering with the ability of tumor cells to multiply and make deoxyribonucleic acid or DNA. Cisplatin is a traditional chemotherapy drug.</p>
<p>Two hundred fifty pleural mesothelioma patients took part in a clinical trial to determine the safety and effectiveness of raltitrexed. One hundred twenty-four patients took cisplatin and the rest took cisplatin plus raltitrexed. Most were men with an average age of 58 who had not received other chemotherapy treatments.</p>
<p>The raltitrexed study was a &#8220;phase III&#8221; clinical trial, as defined by the European Organization for Research and Treatment of Cancer. A phase III trial compares a new cancer treatment with a standard treatment, and requires a large number of participants. It follows a phase II clinical trial, which is concerned with the safety of a new treatment and how to administer it, and a basic phase I clinical trial, which tests the drug&#8217;s effectiveness on a small number of patients.</p>

<p>Survival Increased in Patients Treated with Raltitrexed<br />
 The researchers looked at survival, progression of pleural mesothelioma, safety, tumor response rates, and the patient&#8217;s quality of life. They found the following:</p>
<p>» Forty-six percent of pleural mesothelioma patients in the cisplatin/raltitrexed group survived at least one year, compared to 40% of those in the cisplatin group.</p>
<p>» The average survival was 11.4 months for patients in the cisplatin/raltitrexed group, but only 8.8 months for patients in the cisplatin group.</p>
<p>» Twenty-four percent of patients treated with cisplatin/raltitrexed had a positive response against pleural mesothelioma compared to 14% of patients treated with cisplatin.</p>

<p>» The overall quality of life was about the same in the cisplatin group as compared to the cisplatin/raltitrexed group.</p>
<p>A small number of patients in both the cisplatin and cisplatin/raltitrexed groups developed low white blood cell counts (neutropenia). Other side effects included fatigue, nausea, and vomiting, which were more common in the combined drug group.</p>
<p>Raltitrexed Is Related to the Pleural Mesothelioma Drug Alimta®<br />
 Raltitrexed is in the class of chemotherapy drugs called antifolates. It blocks the production of thymidylate synthase, which in turn decreases the mesothelioma cell&#8217;s ability to make DNA, the basic genetic material involved in cell reproduction. It is registered in Europe for the treatment of advanced colorectal cancer.</p>
<p>Raltitrexed is closely related to pemetrexed (Alimta®), another antifolate. Currently, pemetrexed/cisplatin is the only chemotherapy drug combination that the US Food and Drug Administration has approved to treat pleural mesothelioma.</p>
<p>The authors of the raltitrexed study concluded that combining cisplatin with an antifolate such as raltitrexed or pemetrexed is more helpful in treating pleural mesothelioma than using cisplatin alone. They also felt that cisplatin plus raltitrexed did not have a harmful effect on the patient&#8217;s quality of life.</p>

<p>Malignant pleural mesothelioma is a cancer of the thin layer of tissue that lines the chest and lungs.  It is a rare form of cancer that is mostly associated with exposure to asbestos.  Malignant pleural mesothelioma is a fast-moving cancer with a median survival time of between one and two years.</p>
<p>The majority of mesotheliomas are associated with asbestos exposure.  Asbestos was used widely in the United States for insulation, and while awareness of its harmful effects is increasing, asbestos can still be found in older buildings.  People who work in industries using asbestos in some form, such as insulation manufacturers, insulation workers, and ship builders, may be at increased risk to develop mesotheliomas.  The time between exposure and development of a cancer may be decades, which is why it is usually diagnosed in older people.  More men than women get this type of cancer. <p>Some symptoms of this cancer are increasing breathlessness, pain in the chest or under the ribcage, an abdominal lump or swelling, fever and unexplained weight loss.  Imaging tests like a CT scan may show a collection of fluid in the pleural cavity of the chest.  Pleural tissue and fluid is also tested to detect cancerous cells and certain chemicals that can rule out a diagnosis of pleural mesothelioma.</p>

<p>Diffuse malignant pleural mesothelioma, or DMM, is a rare but highly aggressive type of cancer.  It occurs in the thin layer of tissue, the pleura, lining the chest cavity and lungs.  Once diagnosed with DMM, most patients do not survive beyond one year.  This cancer is more common in men than in women, and in older people.</p>
<p>Most people with DMM have a history of exposure to asbestos; however, if they are not aware that such exposure occurred, they may be unaware that they are at risk.  A patient with DMM typically has one or more of the following signs and symptoms: chest pain, shortness of breath, unexplained weight loss, fatigue, fever, or a cough.  An x-ray usually shows that the pleural tissue around the lungs has thickened due to the development of a number of nodules or small cancerous growths.  Sometimes, there may be a single large growth.  When a biopsy of the cancer tissue is done, the types of cells that make up the cancer may be epithelioid, mixed, or sarcomatoid.  People with epithelioid cell cancers tend to survive somewhat longer; those with sarcomatoid cancers usually have the shortest survival.</p>

Symptoms also tend to present very gradually, and may therefore go unnoticed for a while.  Breathlessness is one of the early and more common symptoms of pleural mesothelioma.  This may present as a recent onset of shortness of breath, or a recent increase in shortness of breath.  Breathlessness may be caused by a buildup of fluid (pleural effusion) in the space surrounding the lungs.  This buildup of fluid may be removed periodically to make breathing easier.  Certain substances may also be introduced into the space between the pleural membranes where fluid collects. <p>Mesothelioma Pleural or Pleural Mesothelioma is generally a severe form of cancer that affects the membrane lining in the thoracic cavity. There are a few benign cases as well. The inhaling of asbestos dust either during the mining or manufacturing or handling of the material or from the systems and applications in which it is used is believed to be cause of this sickness.</p>
<p>The latency period, that is, the time taken for the symptoms to manifest, is anywhere from twenty to forty years. The time lag may even extend to fifty years. Because of this, most patients diagnosed with this disease are in the age group of sixties or seventies. Some may not even remember their exposure to asbestos. By the time the warning signs are noticed and correctly diagnosed, the disease may be in an advanced stage where treatment could be non-effectual.</p>

The usual indications are dyspnea (shortness of breath), persistent coughing, disphagia (difficulty in swallowing), and tightness in chest accompanied by chest pain. Some patients may develop swelling of the neck, face and arms. This may indicate that the mesothelioma has spread. There could also be wheezing, hoarseness, fever and sweating, fatigue, sensory loss and nausea. At the advanced stage there may be hymoptysis (coughing up of blood).</p>
<p>The symptoms are mainly caused by effusion, which in this case means fluid collection in the thoracic area. Also, the delicate membrane covering the lungs may thicken and thus reduce expansion and contraction. In certain cases no perceptible signs may show till the cancer has spread beyond control.</p>

Malignant Pleural Mesothelioma is a lethal cancer that starts in the lining of the lungs. The main cause is believed to be unprotected contact with asbestos. Every year about 3000 new cases of this disease are reported in the United States. It is estimated that over the past fifty years nearly eight million people have been exposed to asbestos and that 300,000 new cases would be reported by the year 2030. The peak may be around 2020 and thereafter the incidence is likely to taper down because of the preventive measures that are being taken.</p>
<p>Like in all cases of cancer, early detection and appropriate treatment improve the survival rate. On both counts the pleural mesothelioma patients are at a disadvantage. Symptoms take anywhere between 20 to 50 years to manifest. Because of this, the patients are generally in the fifty plus age group. And the outwardly noticed indications of the sickness are similar to that of several lesser ailments. This makes the diagnosis difficult. Because of all these, by the time the problem is detected, the cancer is likely to have spread. As yet there is no fully effective line of treatment. It is generally accepted that a combination therapy is better than monotherapy. A great deal of research is being done in this area.</p>
<p>A number of studies have been made about the survival rate among pleural mesothelioma patients. They all come to the same conclusion &#8211; the disease is almost always fatal. The lifespan of a person diagnosed with pleural mesothelioma is about six months to two years. There have been exceptions and their stories are inspiring. The life expectancy varies according to the stage (pleural mesothelioma has four different phases) and the type. One research based on the histologic (tissue structure)tests  shows a median survival of 11 months &#8211; 9.4 months for sarcomatous, 12.5 months for epithelial and 11 months for mixed.</p>

<p>Diagnostic procedure consists mostly of imaging with Computed Tomography (CT scan), Magnetic Resonance Imaging (MRI) and Position Emission Tomography (PET), and laboratory tests and analysis. Thoractomy, which means making an incision through the chest wall to look into the pleura (the membrane lining the lungs and the thoracic cavity) and pleural biopsy provide conclusive evidence.</p>
<p>Diffuse malignant pleural mesothelioma tumor is very aggressive and spreads fast. There are several methods to gauge the stage of the cancer. The most common is known as TNM. T is for the size of the tumor, N for the number of lymph nodules involved, and M for metastasis (spread of the cancer). There are basically four different stages. Correct grading helps the doctors to decide on the course of treatment.  If the affliction is detected at an early stage, the chances of longer survival are better.</p>
